MEDICAL DAY UNIT REFERRAL
You can complete a referral to the Medical Day Unit in the Royal Glamorgan by clicking the picture and logging in
CARDIOLOGY INPATIENT REFERRAL
This form can be used to refer a patient in the Royal Glamorgan Hospital for a inpatient cardiology review
MENTAL HEALTH LIAISON INPATIENT REFERRAL
This form can only be used on the Royal Glamorgan Site to refer to the Mental Health Liaison team
